What is G-REACH?
Funded by a three-year Rural Health Outreach Grant awarded to the Giles Free Clinic by the Health Resources and Services Administration Office of Rural Health Policy (HRSA-ORHP),
G-REACH is a non-emergency health transportation program providing affordable transportation
to low-income residents of Giles County. G-REACH is a product of the early strategic planning
of G-NET, which identified medical transportation as the number one barrier to health care access in Giles County. G-REACH is operated by the Giles Community Health Center and the Giles Health & Family Center.

The types of rides that G-REACH can provide include:
To and from outpatient medical appointments, including behavioral health, dental, vision, and NRV
G-REACH does NOT transport to/from any emergency department or inpatient service.
Rides must be at least one-quarter mile and not over 200 miles one way. Long distance rides may
be shared in order to minimize cost. Shorter rides are allowable for the mobility impaired and
must be approved on a case-by-case basis.
Long distance rides outside of Giles and contiguous counties may only be provided to Giles Community Health Center patients and the appointment must be the result of a referral from the Giles Community Health Center
Long distance rides may be approved for non-Giles Community Health Center patients on a case-by-case basis.
Rides must be reserved at least 24 hours in advance. Reservations placed with less than 24 hours'
notice will be accommodated on a case-by-case basis.
Any ride to/from an appointment at the Giles Community Health Center, even for eligibility determiniation.
Children less than 18 years of age must be accompanied by an adult.
Children less than eight years of age must be restrained in a child safety seat which must be provided by the family.
Door-to-Door service is available upon request.
Persons with mobility impairments (wheelchairs, walkers, canes, etc.) may ride, but must ask for
an accessible vehicle upon scheduling the ride.
Stops for prescription medication pick-up may be made on the return trip ONLY if prescription is
confirmed by the attending practitioner.
To be eligible to use G-REACH, individuals must be:
Residesents of Giles County
Not eligible for Medicaid-covered medical transportation
Not eligible for Agency on Aging senior transportation (some exceptions may be approved if the
Agency on Aging transportation is unavailable on the day of the appointment. Contact the

Not discharged from another transportation service for misuse
Low income (Family income at or below 200% of the federal poverty level; the same criteria as the
Giles Community Health Center. See chart below.)

6:00 a.m.-6:00 p.m. (some exceptions may be approved for after hours)
Fees will vary based on the distance to the destination and may be paid by cash or credit/debit card upon pick-up.
Within Giles County: $5 per round trip
Bland, Craig, Pulaski, Montgomery Counties, VA or Monroe County, WV: $10 per round trip
Long-distance: $0.25 per mile (examples: Roanoke= $36 round trip; UVA Medical Center= $86
round trip; Duke Medical Center= $100 round trip)
